    Personal health record

    A personal health record, or PHR, is a health record where health data and information related to the care of a patient is maintained by the patient. This stands in contrast with the more widely used electronic medical record, which is operated by institutions (such as a hospital) and contains data entered by clinicians or billing data to support insurance claims. The intention of a PHR is to provide a complete and accurate summary of an individual's medical history which is accessible online. The health data on a PHR might include patient-reported outcome data, lab results, data from devices such as wireless electronic weighing scales or collected passively from a smartphone. The term “personal health record” is not new. The earliest mention of the term was in an article indexed by PubMed dated June 1978, and even earlier in 1956 reference is made to a personal health log. However, most scientific articles written about PHRs have been published since 2000. The term "PHR" has been applied to both paper-based and computerized systems; current usage usually implies an electronic application used to collect and store health data.     Microsoft HealthVault is a web-based platform from Microsoft to store and maintain health and fitness information. Started in October 2007, the website is accessible at www.healthvault.com and addresses both individuals and healthcare professionals. In June 2010, Microsoft HealthVault was launched in the UK, the website is accessible at www.healthvault.co.uk A HealthVault record stores an individual's health information. Access to a record is through a HealthVault account, which may be authorized to access records for multiple individuals, so that a mother may manage records for each of her children or a son may have access to his father's record to help the father deal with medical issues. Authorization of the account can be through Windows Live ID, Facebook or a limited set of OpenID providers. An individual interacts with their HealthVault record through the HealthVault site, or, more typically, through an application that talks to the HealthVault platform.     Charlotte Regional Medical Center

    Charlotte Regional Medical Center is a 208 bed for-profit hospital in Punta Gorda, Florida, operated by Health Management Associates of Naples, Florida. It was the first hospital in Charlotte County, and the hospital first opened its doors on August 17, 1947 as Charlotte Hospital. The hospital is mostly known for its cardiology and neurology services. Charlotte Regional Medical Center saw the most damage from Hurricane Charley out of the three hospitals in Charlotte County; damage from the hurricane temporarily disabled all services at the facility. Riverside Behavioral Center is part of Charlotte Regional Medical Center.
